Tax Forms at Libraries

All Madison Public Libraries offer some pre-printed tax forms for free.  Libraries have basic federal and state tax forms available for customers, as well as tax instruction booklets to support some forms. Library staff can also provide customers with the link to online instructions, contact information for WI Dept of Revenue to request that forms and instructions be mailed to you, or can print off forms and instructions on request.  For all prints, the library offers the first five pages free, and charges $.10 per page after.

Free Tax Assistance Sites

Madison Public Library partners annually with other agencies in the community to provide tax assistance. Limited appointments will be available at Central, Lakeview, and Sequoya Libraries in 2023. 

Click here for a list of all 2023 Tax Assistance dates at Central, Lakeview, and Sequoya Libraries, or take a look at our calendar. Appointments are required at all locations. You can call to book an appointment no earlier than January 17, 2023:

 

Library Type of Appointments When you can call
 Lakeview In-person (discuss with your tax preparer and then wait in a separate room until it's done) January 17, 10am FULL, but you can call to get on a wait list
 Sequoya Choice of Drop-off and pick-up (same day) -OR- In-person (sit across from your tax preparer to file your return)

January 23, 9am FULL

 Central In-person (sit across from your tax preparer to file your return)

January 25, 9am (to book a February appt) FULL for February appointments

February 22, 9am (to book an appt in March or April) FULL for March and April

 

Other free or low cost tax preparation sites include: 

  • Richard Dilley Tax Center -- 2238 South Park Street -- by appointment only, January 30th – April 15th, 2023
  • Goodwill of Southcentral Wisconsin(link is external) -- by appointment only. Locations on the North and East sides of Madison, Fitchburg, Portage, Fort Atkinson, Richland Center
  • Middleton Senior Center -- 608-831-2373 -- by appointment only, February 3rd-April 12th, 2023.
  • Monona Senior Center -- 608-222-3415 -- by appointment only.
  • Verona Senior Center -- 608-845-7471 -- by appointment only
