Virtual Library Card Form

1 Start 2 Complete

While many patrons are practicing social-distancing by remaining at home and avoiding crowds, the Madison Public Library has announced a virtual library card to meet the needs of our community during these challenging times. If you don't already have a library card, you can apply for a virtual library card by filling out the form below.

Once the form is submitted, a new library card number will be emailed to you. Your virtual library card provides access to Wisconsin's Digital Library (which is home to more than 71,000 digital books, audiobooks, videos and more) through June 30, 2020, as well as many other online resources, including more ebooks and online magazines purchased through an emergency gift from the Madison Public Library Foundation. Once the library re-opens, users can visit any library location with their photo ID and proof of address to get a full use library card. Please email if you have any questions.

Madison Public Library virtual library cards are only available to people who live in the South Central Library System (Dane, Green, Sauk, Columbia, Adams, Portage, and Wood counties) of Wisconsin.  If you live outside this service area, please contact your local library to see if they are offering a similar resource.

Please choose and enter a 4 digit PIN. This will be used along with your library card number to log in.
