Tax Forms at Libraries

All Madison Public Libraries offer some pre-printed tax forms for free.  Libraries have basic federal and state tax forms available for customers, as well as tax instruction booklets to support some forms. Library staff can also provide customers with the link to online instructions, contact information for WI Dept of Revenue to request that forms and instructions be mailed to you, or can print off forms and instructions on request.  For all prints, the library offers the first five pages free, and charges $.10 per page after.
